Alzheimers Association Chapters in Wisconsin
Wisconsin is supported by three regional organizations responding to specific counties. Each regional organization has its own web site. This link will allow you to select which region serves your county of interest.

The purpose of the Alzheimers Association is to help families cope with all aspects of the disease through information, education, support, and advocacy. Although there is no cure for Alzheimers, there are strategies, therapies, and treatments that can help manage the disease. The Alzheimers Association can be a guide and a support to persons with Alzheimers disease and their families as they learn about options and begin to plan for the future

The Wisconsin Alzheimers Institute (WAI) is an academic center within the University of Wisconsin that represents a private/public partnership between the Helen Bader Foundation, the State of Wisconsin and the University of Wisconsin Medical School.

The Bureau of Quality Assurance responds to two types of health care complaints:

  • Facility Practices —
    Inappropriate or inadequate health care, including but not limited to: facility not following federal or state required policies & procedures; lack of staff training; under staffing; poor quality care; etc.
  • Caregiver Misconduct —
    Specific incident between a caregiver and a resident or patient, including but not limited to abuse (hitting, slapping, verbal, mental, sexual); neglect (intentionally withholding care, failure to carry out a plan of care, disregard of policy); misappropriation (theft of money, identity, credit cars or jewelry, misuse of property, such as using a client's phone without consent).

Consumer Information Reports
The Consumer Information Report is produced annually by the Bureau of Quality Assurance for each Wisconsin nursing home and facility for people with developmental disabilities (FDD - also known as ICF/MR). The report provides information to aid residents or prospective residents in assessing the quality of care provided by a facility.

Facility listings can be had by an alphabetical listing of facilities by the name of the facility or by selecting a county.

What kinds of information is available in these reports? For example:

  • Staff-to-resident ratios
  • Staff retention rates
  • Summary of federal violations
  • Full explanations of the listed data

And, don't forget to check out the State Citations page. This page will allow you to review a facility's state inspection report. The information regarding state citations issued is for facilities surveyed during the period November 1, 2001 through October 31, 2002. Be sure to ask to review the latest survey results when you visit a facility. Facilities without state citations during this period are not listed. Things change, hopefully for the better, you must visit any facility you are considering before you make any decisions about placement. Of course you knew that. After all, you did review my information at choosing a facility -- tell me you did.


Elder Abuse
Wisconsin Statutes mandate each county to provide a publicized phone number to receive reports of suspected elder abuse, and to designate a lead agency for investigation and reporting purposes. Most often, this is the county aging unit or department of social or human services.

Lead agencies must begin investigation of physical abuse, neglect or self-neglect within 24 hours of receiving a report (excluding weekends and holidays) and within five working days of receiving a report of material abuse. Based on the results of its investigation, the lead agency may offer services to help solve the problem.

To report suspected elder abuse, call the nearest Elder Abuse Help Line. You must select your county or tribe from the list provided by the state. A telephone help line has been established in every county by the lead elder abuse agency for reporting suspected cases of elder abuse.

A guardianship is a special relationship between two people, one of whom, called the guardian, acts on behalf of the ward. This relationship may be ordered by a circuit court when it finds that a person is no longer competent to manage his/her affairs. A relative, public official or any other person may petition the court to have a guardianship established. Working with agencies, residents and families, the ombudsman investigates and resolves problems, provides information, referral and consultation and works with licensing, certification, and other enforcement agencies to improve the quality of care in long term facilities and programs.

The ombudsman can help with concerns and problems about:

  • the quality of long term care provided
  • facility residents' rights
  • personal needs allowance
  • physical abuse and neglect
  • dietary needs
  • financial issues (i.e. billing, government benefits, financial exploitation)
  • physical or chemical restraints
  • facility transfer and discharge issues
  • privacy and confidentiality
  • denial of community services

SeniorCare is Wisconsin's Prescription Drug Assistance Program for Wisconsin residents who are 65 years of age or older and who meet eligibility requirements.

Eligibility Requirements Include:

  • Must be a Wisconsin resident
  • Must be 65 years of age or older
  • Must pay a $20 annual enrollment fee per person.
  • Only income is measured. Assets, such as bank accounts, insurance policies, home property, etc., are not counted.

In addition, program participants are subject to certain annual out-of-pocket expense requirements depending on their annual income. The site contains a "pre-application" guide to help you determine you eligibility and approximate your out-of-pocket costs.

Individuals with prescription drug coverage under other health plans will be eligible to enroll in SeniorCare. If you already have a health insurance plan, SeniorCare will coordinate benefit coverage with your plan. Individuals enrolled in Medicaid will not be eligible for the new program, because Medicaid already provides prescription drug coverage.

The Wisconsin Department of Veterans Affairs offers an array of benefits and services to eligible Wisconsin veterans and their families. Additionally, the department operates the Wisconsin Veterans Home at King and the recently opened Wisconsin Veterans Home at Union Grove.
