You can decide -- right now -- what treatment you want or don't want, and you can tell that decision to your doctor and your loved ones so that if you become too sick or unable to tell them, they'll know what you want them to do. Federal law now says that you must also be informed of other ways that you can control the medical treatment you receive. That is the purpose of this pamphlet.

The Division of Long Term Care of the Indiana State Department of Health is committed to being attentive to your concerns about the care and services provided to your friend or family member residing in an Indiana nursing facility. We encourage you to initially alert the administration of the facility of your concerns in an effort to provide the facility opportunity to internally address and correct concerns immediately. If you have done this and feel that further investigation is needed, call our toll-free complaint number.

Indiana's prescription drug plan for low-income seniors. Visit their site for detailed information and download an application in PDF format.

  • You must be 65 years old or older
  • You must be an Indiana resident (living in the state at least 90 days out of the last 12 months).
  • You cannot have prescription drug coverage through an insurance plan, Medicaid or Medicaid with a spend-down
  • Your monthly income must not exceed the following amounts:
  • 997 or less, if single
  • 1,344 or less, if married

Federal regulation/state rule mandates that each facility's most recent annual survey conducted by federal or state surveyors, any plan of correction and any subsequent surveys, be available for public review.

Search by city or county for basic contact information, facility details, and performance data. Statewide averages are provided to give you a reference for looking at the scores of the selected facility. Scores range from 0 to 1579. Scores are considered to be better the closer to zero they are.

  • The top 10% - contains 52 facilities - Scores range from 0 to 16.
  • The top 11% - 25% - contains 79 facilities - Scores range from 17 to 47.
  • The middle 50% range of scores go from 48 to 197 - contains 262 facilities -The Midpoint Score is 97.
  • The lower 11% - 25% - contains 79 facilities - Scores range from 198 to 345.
  • The lower 10% - contains 52 facilities - Scores range from 346 to 1579.

The Senior Health Insurance Information Program is a free & unbiased counseling program provided by the Indiana Department of Insurance for Medicare beneficiaries in Indiana.

SHIIP helps answer your questions about Medicare, Medicare Supplement Insurance, Medicare+ Choice, Medicaid, long term care insurance, prescription coverage and low-income assistance. SHIIP's central office is located in downtown Indianapolis at the Indiana Department of Insurance. The central office only provides counseling services by phone. However, we offer in-person counseling at our local sites which are located throughout the state.

Sites are located throughout the state. Visit one of your local SHIIP sites in order to meet with a counselor in person, or to have questions answered by phone. Here's a directory of local counselors
