Advance Directives - Overview
If you are 19 or older, the law says you have the right to decide about your medical care. If you are very sick or badly hurt, you may not be able to say what medical care you want. If you have an advance directive, your doctor and family will know what medical care you want if you are too sick or hurt to talk or make decisions.

Several organizations have special forms you can use and offer free information about advance directives. Senior citizens may also want to contact the Alabama Department of Senior Services for more information.

Three statewide associations, the Medical Association of the State of Alabama, the Alabama State Bar, and the Alabama Hospital Association, with support from the Alabama Public Health Department and the Alabama Organ Center, have developed an exciting opportunity to deliver an important health care message to all Alabamians.

LIFEPLAN is a statewide campaign to encourage people to discuss health care wishes with their families and document them now, rather than during a crisis.


AgeLine.gov - Support for Family Caregivers (CARES)
Alabama Cares is the state’s version of the National Family Caregiver Support Program, a landmark addition to Older Americans Act services brought to fruition through recommendations by the 1995 White House Conference on Aging. The program is designed to support family caregivers and grandparents raising grandchildren by providing training, assistance and resources to help them take care of their aging loved ones and themselves. This program strives to keep families together and to allow seniors to age at home instead of in costly long term care facilities.

Elder Abuse Information & Prevention
The Alabama Department of Human Resources and the Bureau of Health Provider Standards (BHPS) within the Alabama Department of Public Health have the responsibility for investigating occurrences and allegations of elder abuse. Any reports of abuse (or suspected abuse) received by the ADSS are promptly turned over to the Department of Human Resources and BHPS.

No separate web site was found for the agency responsible. I am assuming the state wants everything to go through this one agency (ADSS) to be dispatched from the central office

SenioRx/Wellness
SenioRx/Wellness is for ALL Alabamians age 55 and over that are prone to chronic diseases or have been diagnosed with chronic diseases requiring daily medications. The program aims to help people manage their chronic illnesses earlier and prevent more serious health problems later in life. ADSS is partnering with the Alabama Department of Public Health to achieve this goal.

SenioRx/Wellness is also a prescription drug assistance program that helps thousands of Alabamians receive free prescription drugs from pharmaceutical manufacturers.

To qualify for free prescription drugs, you must be at least 55 years of age with chronic medical conditions, have no prescription drug insurance coverage and your income must be at or below 200% of the federal poverty level. Volunteers and local coordinators help seniors apply for drug assistance programs provided by pharmaceutical manufacturers.

Alabama has a program to help nursing home residents resolve problems without going to court, but it is vastly understaffed. The state has less than half the ombudsmen needed to provide minimum service to more than 30,000 residents of nursing homes, assisted living facilities and boarding homes, according to a study last year by the Institute of Medicine, an independent health and science research group.


Medicaid - Alabama
Medicaid helps pay medical bills for low-income individuals who meet eligibility criteria. There are three agencies in Alabama that certify individuals for Medicaid. These agencies are:

  • The Social Security Administration,
  • The Department of Human Resources, and
  • The Alabama Medicaid Agency.

Each of these agencies certifies certain groups of individuals for Medicaid based on their circumstances.
Please review Medicaid's General Eligibility Information to determine the appropriate agency to contact.

However( for our purposes:
The Social Security Administration certifies individuals for the following programs: Aged (over age 65) blind) or disabled (according to Social Security standards) persons who have very low income may qualify for cash assistance through the Supplemental Security Income (SSI) program. Individuals eligible for SSI are automatically eligible for Medicaid. To apply for SSI, contact the Social Security Office serving your area or call 1-800-772-1213 (NOTE: "Disabled" means that you must be disabled according to Social Security standards which is: "unable to perform any significant gainful activity, for a period of 12 months or have a condition that will result in death.")


Medicaid Waiver for the Elderly and Disabled
The Medicaid Waiver for the Elderly and Disabled (E & D Waiver) Program is designed to provide services to seniors and the disabled whose needs would otherwise require them to live in a nursing home. Our goal is for clients to retain their independence by providing services that allow them to live safely in their own homes and communities they love for as long as it is appropriate.

Case managers work with clients to develop a plan of care based on the clients’ needs and choices. A good care plan is developed only after careful review of a client’s social, recreational, familial, medical, functional, mental, emotional, and spiritual needs. Individuals in this program have the freedom to choose from a menu of services that will best meet their needs, including: case management, personal care, homemaker, respite, adult day health, companion services and home delivered frozen meals.

Program Highlights

  • The E & D Waiver program currently serves approximately 4,250 clients each year, all of whom are nursing home eligible under Medicaid.
  • E & D Waiver costs the state 18 percent of what nursing home placement would cost for the same number of clients. For example, the average monthly cost per client in fiscal year 2004 was: $3,652 nursing home client $669 E&D Waiver client.
  • Had the 4,250 E & D Waiver clients been in nursing homes in 2004, it would have cost the state approximately $152 million more than it cost for the same clients to be served by the E&D Waiver program.
